Intraocular Lens Implantation in Iran
Intraocular lens (IOL) implantation can be a suitable option for people who are not good candidates for laser vision correction procedures such as LASIK or PRK. It may be used to address a range of refractive errors, including myopia (nearsightedness), hyperopia (farsightedness), astigmatism, and presbyopia (age-related difficulty with near vision).
IOL-based vision correction can generally be performed using two main approaches:
-
Refractive Lens Exchange (RLE)
-
Phakic Intraocular Lens Implantation (PIOL)
Depending on the technique used—such as refractive lens exchange, cataract surgery, or phakic IOL implantation—and the individual patient’s condition, the surgical procedure typically takes around 20 to 30 minutes. The vision correction achieved through these procedures is intended to be long-lasting. In the case of RLE, the eye’s natural lens is removed and replaced with an artificial intraocular lens, so cataracts cannot develop in the natural lens. With a phakic IOL, however, the natural lens remains in place and can develop age-related changes, including cataracts, in the future.
IOLs are manufactured from eye-compatible materials, including specially designed acrylic and silicone materials. These lenses are intended to remain stable inside the eye for long-term use and can provide significant improvement in vision, potentially reducing or eliminating the need for glasses or contact lenses for many patients.
What Are the Different Types of Intraocular Lenses (IOLs)?
Intraocular lenses are available in several designs, allowing ophthalmologists to select an option based on the patient’s visual requirements, lifestyle, and overall eye health.
Monofocal IOLs
Monofocal lenses are designed to provide clear vision at one primary distance, which may be set for near, intermediate, or distance vision. Patients may still need glasses for activities that require a different range of focus, such as reading or working at close range.
Multifocal IOLs
Multifocal lenses are designed with more than one focal point, helping patients see at near, intermediate, and distance ranges. Depending on the patient’s individual visual needs and adaptation, these lenses can reduce reliance on glasses for many everyday activities.
Toric IOLs
Toric lenses are specifically designed for patients with astigmatism. Their optical design compensates for the irregular curvature of the cornea and can improve visual clarity at the distance targeted during treatment. They often reduce the need for glasses.
Accommodative IOLs
Accommodating IOLs are designed to use changes in the eye’s focusing mechanism to provide vision at different distances. Their availability and suitability vary, and they are not appropriate for every patient.
Extended Depth of Focus (EDOF) IOLs
EDOF lenses are designed to extend the range of clear vision, particularly for distance and intermediate activities such as computer use. Some patients may still need reading glasses for fine print or prolonged near work.
The most appropriate IOL depends on several factors, including the patient’s prescription, eye condition, visual expectations, daily activities, and the recommendations of the ophthalmologist following a detailed eye examination.
How Is an IOL Used in Cataract Surgery?
Cataract surgery removes the eye’s natural lens after it has become cloudy and replaces it with an artificial intraocular lens. The purpose of the procedure is to restore clearer vision and, depending on the selected IOL, address certain refractive needs as well.
Cataract surgery is generally performed as an outpatient procedure. The operation itself is relatively short and commonly takes approximately 15 to 30 minutes for each eye, although the total time spent at the medical facility can be longer.
Steps of Cataract Surgery with an IOL
1. Preparing the Eye
The eye is numbed using local anesthesia so that the patient remains comfortable during the procedure. The surgeon then creates a very small opening near the edge of the cornea to gain access to the natural lens.
2. Removing the Natural Lens
The cloudy lens is divided into tiny fragments, commonly using an ultrasound-based technique known as phacoemulsification. The resulting fragments are then gently removed from the eye.
3. Inserting the IOL
The selected intraocular lens is inserted through the small incision using a special injector, usually in a folded form. Once inside the eye, it unfolds and is carefully positioned in the appropriate location, where it is designed to remain stable.
Most patients experience noticeable improvement in their vision during the early recovery period, although the speed of visual recovery varies from person to person. The choice of IOL is made before surgery after considering the patient’s eye measurements, visual goals, lifestyle, and clinical condition.
Who May Not Be a Suitable Candidate for IOL Implantation?
IOL implantation is not suitable for everyone. Eligibility depends on the type of IOL, the patient’s refractive error, corneal and eye health, anterior chamber depth, lens status, and other clinical factors. Patients who have certain eye conditions or do not meet the anatomical and refractive requirements may need another treatment option.
Who May Benefit from Intraocular Lens Implantation?
IOL implantation may be considered for patients with certain levels of myopia, hyperopia, astigmatism, or presbyopia, particularly when laser vision correction is unsuitable or unlikely to provide the desired result. The final decision depends on a detailed eye examination and the patient’s visual needs.
