If you are a Canadian passport holder and you are planning to travel to Iran for low-cost medical treatment, the easiest and fastest way to get a Treatment visa for Iran without spending a lot of money on a guided tour is to apply through a travel agency or medical facilitator companies.
According to the rules passed by the Iranian Ministry of Foreign Affairs, Canadian, British, and American passport holders who travel to Iran (for any touristic or medical purposes) have to come on a guided tour with a prearranged detailed itinerary from arrival to departure and they are not allowed to visit Iran on their own.
You cannot apply for a visa on your own or through the e-visa website. You have to apply for your visa through an official Iranian travel agency. Moreover, as you want to do medical treatment in Iran, you must apply for a medical visa, thus you need to contact an Iranian travel agency and send them your medical records. Then an IPD code will be issued for you and the agency will use this code to get a medical visa for you.
The visa approval will take at least one month. Once approved, you can refer to an Iranian embassy where you have submitted your visa application form for collecting your visa.

If you were bornoutside of Canada and have a passport from Canada, the Iranian Treatment visa or Iranian Tourist visa process will be shorter.
